I've worked with directors and DPs who have used all of these techniques and they're all effective. There is such an intimacy and immediacy with handheld shots. Maintaining high-quality sound while shooting handheld can be a challenge so make sure you have a plan for that as it makes a huge difference.

I really like handheld camera as a cinematic style. Since I'm currently focused on contained location movies, handheld adds a great level of intimacy/presence. I favor either the Easyrig, which allows your DP to shoot long takes and follow the action smoothly; or else handheld at waist level (DP using both hands). I personally prefer this latter height to shoot from rather than shoulder height. I find it's a more interesting angle.
