A former mob doctor risks everything to rescue his daughter from an Appalachian drug lord before she’s ransomed back to the mafia family he betrayed.

SYNOPSIS:

DR. JOHN COOK has run away from the mob and the FBI to a secluded location in Appalachia with his daughter, EVA. EMMETT comes to see his sister, LUANN (Eva’s tutor), at John’s, steals John’s license, and delivers it to CLINT. Clint kidnaps Eva and delivers her to COTTON SWAIN, a drug kingpin in Appalachia. Cotton gives John the ultimatum to turn himself over to him to be ransomed back to the mob or Eva goes in his place. Luann convinces John that if he gives himself up they will send both him and Eva to his former bosses. He realizes the only way they remain safe is to rescue Eva himself.

John calls in a favor from an old mob friend, MICKEY. The two of them, along with John’s seven dogs, go in search of Cotton’s compound. They rough up TUCKER, a local drug dealer, for information about how to move up the food chain. Meanwhile, Cotton’s cronies: JIMMY, BRYSON, and DIP hold Eva captive. The new head of John’s former mob family, VINCENT LEONE, tells his sister MIA, John’s ex, to go to Appalachia and await delivery of John from Cotton Swain.

John and Mickey confront TOBIAS, the next criminal up. Tobias is killed by John’s dogs before he can give up any useful info, but John finds the name of what is known as “Black Mountain” on Tobias’ phone. Micky and John arrive at the mountain and break into one of Cotton’s meth labs. As they interrogate the CHEF, Bryson and Dip arrive. Bryson kills one of John’s dogs, Tuesday, and Dip nearly blinds Mickey. John breaks free and kills Dip while his dogs take care of Bryson. A roughed-up Micky abandons John.

John decides that he must turn himself in and calls FBI AGENT HUDSON. They strike a deal that John will give them the information and testimony against the mob if the FBI saves his daughter. Meanwhile, Cotton has raised the price on the Leone family, and Mia decides to contact mob members from Louisville. As the FBI make their way to the mountain, Cotton’s local cops begin firing on them. John gets Clint and tries to infiltrate Cotton’s compound. Mia and the mafia show, but Cotton only offers them Eva.

John is captured by LOUISVILLE LOUIS and taken before Mia. Cotton and Mia’s people argue over money, and it turns violent. Mia kills Cotton. Mickey returns and saves John. Mia kills Mickey. John kills Mia. Months after these events, a cleaned-shaven John shows up in court to testify against the mob, leaving Eva with Luann and the safety of the FBI’s witness protection program.
